My sincere condolences to you and your daughter….I am very sorry and sad to hear about Jack’s passing. I have known Jack as an attentive and compassionate co-worker. Through our check in questions in our daily huddle, I got to know Jack better and we shared some good laughs. He was very proud of his family. I wish you strength in this difficult time.

I had the pleasure of working with Jack doing outreach mental health in 2017/2018. I shared a desk with him for two years working along side him through the ACT Team. I don’t think a day went by where Jack didn’t share some of his knowledge or ask questions of others. I will remember him as a kind man who sought out knowledge and understanding. When I came on the team he took time out of his busy days in the first few weeks to make me feel comfortable. As I left the team to another one in IH, he went out of his way to seek me out, offer a future reference, gave me his number, and said some extremely nice things to me. In hindsight. it was him mentoring me and a reflection of how Jack chose to carry himself; in a professional manner and with kindness. He cared deeply about his colleagues, his clients and their outcomes. He was always keen to learn and share information. He was passionate about giving back to others and a deeply caring man. You felt it in the way he would say good morning, in how he carried himself, how the talked of his clients and the emotions he would carry when we would deal with the unfortunate realities of death within mental health/addictions. I will remember Jack as a caring and consistent man. You always knew what you got with him and I respect that quality in him.
Sorry to hear of your family loss. Sending some healing thoughts and positive memories of Jack. Rest in peace friend.
I had known Jacek almost 50 years ago. I met him when I was 15 and he was 25. He was my first boyfriend – had always had many interests and excelled in music and sports. He introduced me to Leonard Cohen’s songs and I am forever grateful for it. I am sorry he died so early in his life. He took great care of health and had come from long lived family.
